1. Download the required registration forms below.
2. Pay fees on or before due date (payable to Club Italia).
NEW PLAYERS: In addition to the above registration requirements, you will
need one color passport-size photo that fits within the box on the registration
form. Fill out the form completely. Don't forget to sign in the box, and check
the AMATEUR box. Leave the top line blank.
